Appalachia Trip Forms and Information

The Appalachia Group is preparing for its 15th annual trip to Preston County, WV to volunteer their time and talents to  the summer work program thru the Catholic Church of Preston County.  Below are the forms and information needed to participate.

 All forms must be returned by June 7, 2010
A box marked "Appalachia" will be in the vestibule of church for your convenience.  Or forms can be given directly to Rose, Charlotte of Trish.

Booklet for ages 18 and older to read and familiarize themselves with. The Catholic Church of Preston County will hold a discussion at the orientation when we arrive in WV for our week long stay. After the discussion, those 18 and older will be asked to sign an acknowledgement:

The forms below are to be filled out by everyone going on the trip regardless of age. If an entire family is going or more than one person from a family, then one parish overnight registration form will suffice.

Form Instructions:

  • The person should put as the name of the event:
    Catholic Church of Preston County 2010 Summer Home Repair Program

  • the date of the event:
    Sun. June 20 - Fri. June 25

  • For name:
    list ALL names of family members attending along with their age and sex
    school and grade can be listed as N/A
    birth date should also be listed for each individual

  • in the permission section:
    the attendee should put in 6/20 - 6/25

  • If the only person going is 18 or older, they can sign for themselves where it says Parent/Guardian Signature

  • if the person going is under 18 and no parent or guardian is going, then their parent or guardian must sign

  • The other information must also be filled in regardless of age.

  • A consent to treat form must be filled out for everyone, regardless of age!
    If the person going is 18 or older, they will sign for themselves.
    And again if under 18 and no parent going, the parent must sign.

  • The Code of Conduct form must be signed by everyone regardless of age. The code will be discussed before going to WV for the week.

Rules for the Trip:

  • The Summer Home Repair Program should be read by everyone because it will be discussed at our orientation at St. Sebastian Catholic Church the evening of our arrival in WV. 

  • St. Joseph the Worker Youth Camp Rules should be read by everyone. This is where we will stay for the week and the rules will be reviewed upon arrival at the camp.
